Delta Dental of Iowa Foundation Announces $40,000 Community Change Grant to Linn County Public Health

Linn County Public Health (LCPH) received a $40,000 Community Change Grant from Delta Dental of Iowa Foundation. The grant is intended to further the work of My Care Community (MCC), an integrated community care coordination system of more than 35 cross-sector agencies and organizations. Delta Dental’s Community Change Grants fund innovative projects with a goal of addressing key community health issues. 

Improving the health of our communities requires intentional partnerships and cooperation to support all sectors and systems such as health care, public health, education, housing, social services and more. Social determinants of health can impact an individual’s overall health and well-being and all systems must work together to support solving the conditions that exist. In 2016, MCC was founded by a State Innovation Model grant with a mission to improve the quality of life, health outcomes and social determinants of health of the community.

“We’re proud to support the important work of LCPH, and all of the agencies and nonprofits involved who are committed to the success of MCC,” said Suzanne Heckenlaible, executive director, Delta Dental of Iowa Foundation. “Our Community Change Grant will assist MCC to transform how existing systems come together to improve access to services and improve the health and overall wellness of individuals.”

“Creating an equitable care coordination system between the health and human-services sector requires implementing, maintaining and enhancing many moving pieces among numerous partners to be successful,” said Dr. Pramod Dwivedi, director, Linn County Public Health. “This $40,000 grant will allow us to continue making the necessary adjustments so that community members who historically struggled to access services will be able to connect to agencies in a timely and efficient manner. In addition, partners will measure the progress of clients towards better health, education, and financial stability, and community leaders will have access to accurate, reliable, and unbiased data for individual and systems-level decision making.”
